School, sports & work physicals

Need a form filled out before your child attends school or camp? Required by your employer to get a physical exam? Novant Health Family Medicine Residency Clinic completes these specialized exams:

• School physicals.
• Kindergarten physicals.
• Sports participation physicals.
• Camp physicals.
• Work physicals.

These exams help to determine if your health could put you or others at risk if you participate in a specified activity. Many of these required exams can be completed at the same time as your annual checkup. Some might require additional testing, and could incur an additional cost. Contact your insurer before making your appointment to see if your exam will be covered.

If you or your child have been given a form to complete, bring that with you to your appointment as we will not have access to it in our office.
