New patient information

Novant Health Family Medicine Residency Clinic welcomes new patients. To make your first appointment with a provider in our clinic:

• Call us at 704-316-5170 between 8 a.m. and 5 p.m. Monday through Friday.
• Use MyChart to schedule anytime online.

Bring with you to every appointment:

• Your photo ID.
• The insurance card covering you or your child as well as your method of copayment or payment in full. (Click here for more insurance and payment information.)
• Any work, school, sports or camp forms you need completed for yourself or your child.
• Any medications currently being taken by you or your child, including over-the-counter vitamins and supplements.
